Objetivo: A pesquisa visa determinar o perfil bioquímico e sorológico das hepatites B e C em internos de um centro de recuperação, Ananindeua, Pará, Brasil. Métodos: Estudo transversal, descritivo e quantitativo, desenvolvido entre 2015 e 2018. Os dados foram coletados com o uso de Ficha de Inquérito e entrevista. Os participantes foram submetidos à coleta de sangue para realização de testes sorológicos para as hepatites virais B e C e bioquímicos. Resultados: Participaram 125 internos, com frequência de 97,6% para o sexo masculino, prevalecendo a faixa etária de 31 a 40 anos (38,4%). Os marcadores bioquímicos que mais sofreram alterações: ácido úrico, alanina aminotransferase e lipoproteína de alta densidade. O HBsAg não foi detectado, porém houve detecção de anti-HBc total reagente isolado em 1,6% dos indivíduos. Em 20,8% pode-se observar resposta vacinal contra o vírus da hepatite B. A pesquisa detectou prevalência de 3,2% de anti-VHC reagente. Conclusão: É baixa prevalência da infecção pelos vírus das hepatites B e C, apesar dessa população ser considerada de elevado risco para a transmissão desses vírus, os examinados na sua maioria referiu utilizar apenas drogas inaláveis. A baixa cobertura vacinal encontrada entre os examinados demonstrou a vulnerabilidade em adquirir a hepatite B e a importância de estudos entre usuários de drogas no Pará. (AU)


Objective: The research aims to determine the biochemical and serological profile of hepatitis B and C in inmates of a recovery center, Ananindeua, Pará, Brazil. Methods: Cross-sectional, descriptive and quantitative study, developed between 2015 and 2018. Data were collected using an Inquiry Form and an interview. Participants underwent blood collection to perform serological tests for viral hepatitis B and C and biochemicals. Results: 125 inmates participated, with a frequency of 97.6% for males, with the age group of 31 to 40 years old prevailing (38.4%). The biochemical markers that suffered the most changes: uric acid, Alanine aminotransferase and High density lipoprotein. HBsAg was not detected, but total anti-HBc reagent isolated was detected in 1.6% of individuals. In 20.8%, a vaccine response against the hepatitis B virus can be observed. The survey found a 3.2% prevalence of anti-HCV reagent. Conclusion: The prevalence of infection by the hepatitis B and C viruses is low, although this population is considered to be at high risk for the transmission of these viruses, the majority of those examined reported using only inhalable drugs. The low vaccination coverage found among those examined demonstrated the vulnerability to acquire hepatitis B and the importance of studies among drug users in Pará. (AU)


Objetivo: La investigación tiene como objetivo determinar el perfil bioquímico y serológico de la hepatitis B y C en los reclusos de un centro de recuperación, Ananindeua, Pará, Brasil. Métodos: Estudio transversal, descriptivo y cuantitativo, desarrollado entre 2015 y 2018. Los datos se recopilaron mediante el Formulario de encuesta y la entrevista. Los participantes se sometieron a extracción de sangre para pruebas serológicas de hepatitis viral B y C y bioquímicos. Resultados: Participaron 125 reclusos, con una frecuencia del 97,6% para los hombres, prevaleciendo el grupo de edad de 31 a 40 años (38,4%). Los marcadores bioquímicos que sufrieron más cambios: ácido úrico, Alanina aminotransferasa y Lipoproteínas de alta densidad. No se detectó HBsAg, pero se detectó el reactivo anti-HBc total aislado en el 1,6% de los individuos. En 20.8%, se puede observar una respuesta de vacuna contra el virus de la hepatitis B. La encuesta encontró una prevalencia del 3.2% Del reactivo anti-VHC. Conclusiones: La prevalencia de infección por los virus de la hepatitis B y C es baja, aunque se considera que esta población tiene un alto riesgo de transmisión de estos virus, la mayoría de los examinados informaron que usaban solo medicamentos inhalables. La baja cobertura de vacunación encontrada entre los examinados demostró la vulnerabilidad a contraer hepatitis B y la importancia de los estudios entre usuarios de drogas en Pará. (AU)

ABSTRACT Objective: The national vaccination coverage survey on full vaccination at 12 and 24 months of age was carried out to investigate drops in coverage as of 2016. Methods: A sample of 37,836 live births from the 2017 or 2018 cohorts living in capital cities, the Federal District, and 12 inner cities with 100 thousand inhabitants were followed for the first 24 months through vaccine record cards. Census tracts stratified according to socioeconomic levels had the same number of children included in each stratum. Coverage for each vaccine, full vaccination at 12 and 24 months and number of doses administered, valid and timely, were calculated. Family, maternal and child factors associated with coverage were surveyed. The reasons for not vaccinating analyzed were: medical contraindications, access difficulties, problems with the program, and vaccine hesitancy. Results: Preliminary results showed that less than 1% of children were not vaccinated, full coverage was less than 75% at all capitals and the Federal District, vaccines requiring more than one dose progressively lost coverage, and there were inequalities among socioeconomic strata, favorable to the highest level in some cities and to the lowest in others. Conclusion: There was an actual reduction in full vaccination in all capitals and the Federal District for children born in 2017 and 2018, showing a deteriorating implementation of the National Immunization Program from 2017 to 2019. The survey did not measure the impacts of the COVID-19 pandemic, which may have further reduced vaccination coverage.


RESUMO Objetivo: Inquérito nacional de cobertura vacinal aos 12 e 24 meses de idade foi realizado para investigar as quedas nas coberturas a partir de 2016. Métodos: Amostra de 37.836 nascidos vivos das coortes de 2017 e 2018 residentes nas capitais, Distrito Federal (DF) e 12 cidades com mais de 100 mil habitantes, acompanhados nos primeiros 24 meses por registros nas cadernetas de vacinação. Setores censitários foram estratificados segundo condições socioeconômicas, e o mesmo número de crianças foi incluído para cada estrato. Calcularem-se coberturas vacinais de cada vacina e coberturas completas aos 12 e 24 meses, doses aplicadas, válidas e oportunas. Fatores familiares, maternos e da criança associados à cobertura foram pesquisados. Os motivos para não vacinar analisados foram: contraindicações médicas, dificuldades de acesso, problemas no funcionamento do programa e hesitação vacinal. Resultados: Os resultados preliminares mostram que menos de 1% das crianças não foram vacinadas, as coberturas pelo esquema completo são menores que 75% em todas as capitais e no DF, as vacinas com mais de uma dose perdem cobertura progressivamente, há diferenças entre os estratos socioeconômicos, favoráveis aos estratos mais altos em algumas cidades e aos estratos mais baixos em outras. Conclusão: Houve realmente redução da cobertura vacinal em todas as capitais e no DF para as crianças nascidas em 2017 e 2018, denotando piora na execução do Programa Nacional de Imunizações durante os anos de 2017 a 2019. O inquérito realizado não mensurou os impactos da pandemia de COVID-19 que podem ter reduzido ainda mais as coberturas vacinais.

Ante el riesgo real de ocurrencia de brotes de parálisis fláccida aguda en la región debidos a poliovirus derivado de la vacuna Sabin o a la importación de poliovirus salvaje, la Sociedad Latinoamericana de Infectología Pediátrica comisionó a un grupo ad hoc de expertos integrantes del Comité de Vacunas y Biológicos de la institución, para redactar un documento oficial de posición sobre la necesidad imperiosa de incrementar los niveles de inmunización contra la enfermedad en la región e incorporar definitivamente en forma exclusiva la vacuna de polio inactivada en todos los esquemas nacionales de vacunación. La presente publicación discute las principales conclusiones y recomendaciones generadas como resultado de esta actividad.

This publication discusses the main conclusions and recommendations generated as a result of such activity.

Introdução: Hesitação vacinal é o atraso em aceitar ou a recusa das vacinas recomendadas. A Organização Mundial da Saúde recomenda uma cobertura vacinal por volta de 95% para garantir os benefícios da vacinação. Apesar disso, a hesitação em vacinar é crescente, o que deixa parte da população suscetível a doenças imunopreveníveis e favorece o ressurgimento de enfermidades já erradicadas. O objetivo do estudo foi identificar a hesitação vacinal e fatores associados a esta em um ambulatório-escola no sul de Santa Catarina. Métodos: Estudo observacional transversal realizado em ambulatório-escola no período de outubro a dezembro de 2019. Resultados: O estudo foi composto por 221 responsáveis por crianças e adolescentes até 16 anos. Apesar de todos os responsáveis exporem que vacinam os seus filhos, 84,2% estavam com a carteira de vacinação atualizada. A prevalência de hesitação vacinal foi de 20,09%, e os principais motivos destacados foram a preocupação com os efeitos adversos e a falta de segurança em realizá-las. Em relação à intensidade da hesitação, 52,77% deixaram de aplicar somente uma vacina recomendada, e 41,66% vacinaram, mesmo hesitando. Entre aqueles que já hesitaram, 65,62% não imunizaram seus filhos contra o vírus Influenza. Conclusão: Apesar de a maioria vacinar seus filhos, a taxa de hesitação encontrada não pode ser ignorada, devendo ser revertida. Para isso, se faz necessário o desenvolvimento de programas e campanhas informativas para o esclarecimento da população local frente à importância e aos benefícios da vacinação.


Introduction: Vaccine hesitancy is the delay in acceptance or refusal of recommended vaccines. The World Health Organization recommends around 95% vaccination coverage to ensure the benefits of vaccination. Despite this, hesitancy to vaccinate is increasing, leaving part of the population susceptible to immunopreventable diseases and favoring the resurgence of already eradicated diseases. This study aimed to identify vaccine hesitancy and associated factors in a school outpatient clinic in southern Santa Catarina. Methods: The study carried out a cross-sectional observational study in a school outpatient clinic from October to December 2019. Results: The 221 guardians of children and adolescents up to 16 years composed the research. Although all guardians exposed that they vaccinate their children, 84.2% were with the vaccination cards updated. The prevalence of vaccine hesitancy was 20.09%, and the main reasons highlighted were concern about adverse effects and lack of safety when vaccinating. Concerning the intensity of hesitancy, 52.77% failed to apply only one recommended vaccine, and 41.66% were vaccinated even though they hesitated. Among those who hesitated, 65.62% did not immunize their children against the influenza virus. Conclusions: Although the majority vaccinate their children, the hesitancy rate found cannot be ignored and should be reversed. For this, it is necessary to develop informative programs and campaigns to educate the local population about the importance and benefits of vaccination.

ABSTRACT

Background: In each geographic region, risk of new cases of COVID19 are driven by internal factors such as agent, host, and environment characteristics, as well as external factors, such as population mo-bility and cross border transmission of disease. COVID19 control measures are best implemented when local governments and health teams are well aware of these internal and external risks. These risks are dynamic in nature and hence need to be reviewed at regular intervals. The study conducted to develop a composite spatiotemporal Hazard Index comprising of three factors – presence of susceptible popula-tion, population density and presence of active cases with corresponding growth rates, to rank areas within an administrative boundary by their fortnightly risk of active COVID19 cases. Methods: Using Principal Component Analysis, the weights of each of these factors were determined and applied to transformed values of factors in the districts of Gujarat state for months of January to July 2021. Hazard Index thus obtained was used to rank the districts. Results: Spearman correlation between the Hazard Index and number of active cases 15 days later was moderate and significant (p<0.01) throughout the study period. Conclusion: Hazard Index can predict Districts at highest risk of active cases in the given time period. These districts with high Hazard Index would require different control measures, depending on the fac-tor that resulted in higher index value.

@#Abstract: Objective To analyze the serotype distribution, drug resistance rate and drug resistance gene carrying of Streptococcus pneumoniae isolates in hospitalized patients, and evaluate the coverage of the vaccine to the serotype of Streptococcus pneumoniae in this area, so as to provide reference for the rational use of antibiotics in clinic. Methods A total of 150 strains of non-repetitive Streptococcus pneumoniae isolated from inpatients from January 2015 to December 2019 were collected for serotyping and antimicrobial sensitivity test. The carrying rates of pbp2b, ermB and tetM were detected by PCR. Results The PCR classification rate of 150 strains of Streptococcus pneumoniae was 93.1%, and the classification rate of capsular swelling test was 100%, and a total of 19 serotypes were divided, mainly 19F and 6B. Children's serotypes were predominantly 19F, 6B, and 15A; adult serotypes were predominantly 19F, 14, and 23F. The coverage rates of the PCV7, PCV10, PCV13 and PPV23 vaccines were 36.8%, 42.1%, 57.9% and 68.4%, respectively. Strains with serotypes of 19F, 6B, 3, and 23F had higher rates of resistance to antimicrobials. The sensitivity of Streptococcus pneumoniae to penicillin was greater than 96.0%. Antimicrobials with significant differences in resistance rates between invasive and non-invasive strains were penicillin, moxifloxacin, and levofloxacin. The percentage of strains carrying both ermB and tetM resistance genes was 96.0%, and the concordance rate between pbp2b, ermB and tetM resistance genes and the resistance phenotype was >98.0%. A total of 10 multi-resistance combinations were detected, with a multi-resistance rate of 62.6%, and the multi-drug resistance pattern of Streptococcus pneumoniae was mainly concentrated in the 19F and 6B serotypes. Conclusion There are significant age differences in the serotypes of Streptococcus pneumoniae in this area. The vaccine currently used has low coverage in this region and therefore offer limited protection to the population. The drug resistance rates of Streptococcus pneumoniae varied significantly among serotypes. Erythromycin and tetracycline are not recommended for clinical treatment of Streptococcus pneumoniae. Penicillin can still be used as the first choice for clinical treatment of Streptococcus pneumoniae infection.

Introduction@#Diarrhea is defined as a person excretes more than three times in 24 hours with pathological impurities of more than 10 mg/kg per day. According to the fact sheets of the World Health Organization in 2019, diarrhea is the second leading cause of death among children under 5 years of age. Researchers suggest that about 50 percent of infantile diarrhea occurs in temperate countries and it reaches almost 80 percent in winter which is mainly caused by rotavirus. While immunization is the most effective way to prevent rotavirus infection, there were two types of rotavirus vaccines that have been licensed and available on the global market since 2006. Rotavirus immunization in young children is a safe and effective public health method for controlling rotavirus infection which therefore can reduce childhood morbidity and mortality.@*Study aim@#To study the incidence, clinical manifestations, and complications of rotavirus among children hospitalized with acute diarrhea.@*Methodology@#The study will be conducted using the observational method including descriptive analysis. Statistical data for 2018-2020 will be obtained and analyzed from the pediatric wards of the “Gurvan Gal” hospital. Children diagnosed with rotavirus diarrhea who meet the criteria to be included in the study will be selectively sampled with further analysis of the incidence, clinical features, toxicity, and dehydration of acute diarrhea according to the medical history.@*Results@#Universal immunization is important to significantly reduce rotavirus-associated diarrhea, thereby reducing infection and the risk of disease in infants and young children.

INTRODUCTION: with the current situation of measles outbreak in Brazil and the sharp drop in immunization coverage nationwide, it is necessary to think about ways to intervene in the face of so many difficulties, since the expansion of immunization coverage is the most effective and safe for the control of infectious diseases OBJECTIVE: to analyze vaccine coverage of two National Campaigns during the COVID-19 pandemic in Vitória/ES. METHODS: a descriptive experience report on the implementation of an intervention plan to expand vaccination coverage of two National Vaccination Campaigns in 2020 in a Family Health Unit (FHU) during the Curricular Internship I (EC-I). It was based on the Situational Strategic Planning. Descriptive and bivariate statistical analyzes were performed RESULTS: there was an increase of 5.79 times in the vaccination coverage of the National Measles Vaccination Campaign 2020 in the territory, from 0.86% (vaccination coverage of the triple viral before the start of EC-I) to 4.98% at the end of EC-I (p = 0.438). When comparing the vaccination coverage of the National Multivaccination Campaign 2020 of immunobiologicals administered before and after the EC-I period at the FHU, we observed that there was a statistically significant difference (p = 0.0049). CONCLUSION: the proposed and implemented intervention plan had a positive outcome on the FHU, as the vaccination coverage of the two campaigns were expanded, despite all the challenges of the pandemic


INTRODUÇÃO: com a conjuntura atual do sarampo no Brasil, além da queda acentuada da cobertura vacinal em âmbito nacional, faz-se necessário pensar sobre as formas de intervir frente a tantos desafios, compreendendo que a ampliação da cobertura vacinal é a forma mais efetiva e segura para o controle de doenças infectocontagiosas. OBJETIVO: analisar a cobertura vacinal de duas Campanhas Nacionais durante a pandemia de COVID-19 em Vitória/ES. MÉTODO: relato de experiência descritivo, sobre a implementação de um plano de intervenção de ampliação de cobertura vacinal de duas Campanhas Nacionais de Vacinação em 2020 em uma Unidade de Saúde da Família (USF) durante a Disciplina de Estágio Curricular I (EC-I). O plano de intervenção foi alicerçado no Planejamento Estratégico Situacional. Análises estatísticas descritivas e bivariadas foram realizadas. RESULTADOS: houve um aumento de 5,79 vezes da cobertura vacinal da Campanha Nacional de Vacinação contra o Sarampo 2020, passando de 0,86% (em março de 2020) para 4,98% no final de outubro de 2020 (p=0,438). Ao comparar a cobertura da Campanha Nacional Multivacinação 2020 dos imunobiológicos administrados antes e após o plano de intervenção, observamos que houve uma diferença estatisticamente significante (p=0,0049). CONCLUSÃO: verificou-se um aumento nas coberturas vacinais da Campanha Nacional de Vacinação contra o Sarampo 2020 e da Campanha Nacional Multivacinação 2020. O plano de intervenção proposto e implementado surtiu efeito positivo na USF, pois, foi ampliado a cobertura vacinal das duas Campanhas, mesmo com todos os desafios da pandemia.

RESUMEN Introducción: La pandemia de COVID-19 produjo una crisis sanitaria afectando las coberturas de vacunación de los países. Objetivo : Describir la cobertura de vacunación de los biológicos trazadores durante el periodo prepandémico (2015- 2019) y pandémico (2020-2021) en Paraguay. Materiales y Métodos: Estudio descriptivo observacional de corte transverso, que comprendió a niños de 0 a 5 años de edad, de las 18 Regiones Sanitarias del País. Se analizaron las coberturas de vacunación del Programa Ampliado de Inmunización, periodo prepandémico (2015-2019) y pandémico (2020-201). Se incluyeron los biológicos trazadores: BCG, DPT1, DPT3, IPV1 y bOPV3, SPR1, SPR2 y vacuna antiamarílica (AA), el cálculo de cobertura de vacunación a nivel país se realizó por medio del análisis del reporte electrónico semanal de dosis de vacunas administradas por Región Sanitaria. Se estableció la comparación de las coberturas de vacunación por biológico trazador, por periodos y rango de edad. Resultados: Durante la pandemia se constató un descenso de cobertura de vacunación de los biológicos trazadores, para BCG: 4 % (2020), 15% (2021);DPT1: 5 % (2020), 13 % (2021); DPT3: 9 % (2020), 22 %(2021); IPV1: 5 % (2020),16 % (2021); bOPV3: 7% (2020),19 % (2021); SPR1: 9 %, SPR2: 13 % (2020),SPR1:17%, SPR2:16 % (2021) y AA: 7% (2020), 15% (2021). Conclusión: Durante la pandemia de Covid-19 disminuyeron las coberturas de vacunación de todos los biológicos trazadores, similar descenso se constató en otros países de América, existe el riesgo de aparición de brotes de enfermedades prevenibles por vacunación por el acúmulo de susceptibles.


ABSTRACT Introduction: The COVID-19 pandemic produced a health crisis affecting countries' vaccination coverage statistics. Objective: To describe the coverage of recommended vaccines during the pre-pandemic (2015-2019) and pandemic (2020-2021) periods in Paraguay. Materials and Methods: This was a descriptive, observational cross-sectional study, comprising children from 0 to 5 years of age, from the Country's 18 Health Regions. The vaccination coverage of the National Expanded Immunization Program, pre-pandemic (2015-2019) and pandemic (2020-201) periods were analyzed. The recommended vaccinations included were: BCG, DTP1, DTP3, IPV1 and bOPV3, MMR1, MMR2 and yellow fever vaccine (AA), the calculation of vaccination coverage at the country level was carried out through the analysis of the weekly electronic report of doses of vaccines administered by Health Region. The comparison of vaccination coverage by recommended vaccine was established, by periods and age range. Results: During the pandemic, a decrease in vaccination coverage of the recommended vaccines was observed, for BCG: 4% (2020), 15% (2021); DTP1: 5% (2020), 13% (2021); DTP3: 9% (2020), 22% (2021); IPV1: 5% (2020), 16% (2021); bOPV3: 7% (2020), 19% (2021); MMR1: 9%, MMR2: 13% (2020), MMR1: 17%, MMR2: 16% (2021) and AA: 7% (2020), 15% (2021). Conclusion: During the Covid-19 pandemic, vaccination coverage of all recommended vaccines decreased, a similar decrease was found in other countries in the Americas, there is a risk of outbreaks of vaccine-preventable diseases due to the accumulation of susceptible populations.

Resumen Desde 1982, cada año el Departamento de Inmunizaciones del Ministerio de Salud de Chile lleva a cabo la campaña de vacunación contra influenza junto con las Secretarías Regionales Ministeriales-SEREMI, Servicios de Salud y centros de atención primaria de salud. Con los objetivos de prevenir mortalidad y morbilidad grave en grupos de mayor riesgo y de preservar la integridad de los servicios de salud, hasta el 2020 las campañas de vacunación contra influenza serían las más grandes implementadas en Chile, para dar paso, el 2021, a la vacunación contra SARS-CoV-2. Obedeciendo a cambios demográficos y epidemiológicos locales y acogiendo los avances científicos sobre seguridad e inmunogenicidad de la vacuna, el incremento de las vacunas influenza disponibles en Chile forma parte de la planificación anual de la campaña. El 2020, sin embargo, la Campaña Influenza tuvo que ser re-planificada en curso como consecuencia de la incorporación de nuevos grupos a vacunar según dispuso la modificación de la alerta sanitaria por brote de SARS-CoV-2 del 6 de marzo de 2020. Así, de 6.799.800 de dosis, el Departamento de Inmunizaciones logró en menos de dos meses aumentar la disponibilidad a 8.480.325, y cumplir con el compromiso de garantizar el acceso de los grupos de riesgo al beneficio de la vacunación estatal gratuita.


Abstract In Chile, the Immunization Department of the Ministry of Health has carried out the seasonal influenza vaccination campaign annually since 1982 in collaboration with the national health services, regional health offices, and primary health care centres. With the aim of preventing deaths and serious morbidity in high-risk groups and preserving the integrity of health services, the seasonal influenza campaign had been the largest implemented in Chile until 2020, since in 2021 the vaccination campaign against SARS-CoV-2 is expected to become the largest ever implemented. In response to local demographic and epidemiological changes, and taking into account the new scientific evidence on the safety and immunogenicity of vaccines, the influenza vaccines available in Chile would increase annually as a result of campaign planning. In 2020, the influenza campaign had to be re-planned while in progress due to the addition of new high-risk groups to be vaccinated in accordance with the SARS-CoV-2 pandemic health alert modification of March 6th, 2020. Over the course of three weeks, the Immunization Department managed to increase the doses of available influenza vaccines from 6,799,800 previously agreed upon to 8,480,325 and thus serve high-risk groups, guaranteeing their access to state funded influenza vaccination.

Objetivo: Analisar as coberturas vacinais de crianças menores de 1 ano em Roraima, Brasil, entre 2013 e 2017, e expor as percepções dos profissionais de saúde quanto às barreiras que influenciaram no alcance de elevadas coberturas vacinais do estado em 2017. Métodos: Estudo descritivo que analisou as coberturas vacinais para bacilo de Calmette e Guérin (BCG), rotavírus, poliomielite, febre amarela, pentavalente, meningocócica conjugada C e pneumocócica 10-valente, baseado em dados dos sistemas de informações de imunizações. As barreiras percebidas pelos profissionais foram mensuradas por questionário com respostas em escala de Likert. Resultados: A maior cobertura foi da BCG (146,1%) em 2014; e a mais baixa, da vacina contra o rotavírus (70,4%) em 2013. A principal barreira (56/100) identificada pelos profissionais atuantes no programa (100 respondentes) para melhores coberturas foi a dificuldade de acesso à internet. Conclusão: As baixas coberturas vacinais refletem a influência das barreiras de acesso a vacinação.

Métodos: Estudio descriptivo que analizó las coberturas para el bacilo de Calmette y Guérin (BCG), rotavirus, poliomielitis, fiebre amarilla, pentavalente, meningococo C conjugado y neumocócica 10 valente, basado en datos de los sistemas de información de inmunizaciones. Las barreras percibidas por los profesionales se midieron en un cuestionario basado en la escala Likert, por entrevista o autoaplicación. Resultados: La mayor cobertura fue de BCG (146,1%) en 2014; y la más baja, de rotavirus (70,4%) en 2013. La principal barrera (56/100) identificada por los profesionales que trabajan en el programa (100 participantes) para una mejor cobertura fue la dificultad de acceso a la internet. Conclusión: La baja cobertura de vacunación refleja la influencia de las barreras para acceder a la vacunación.


Objective: To analyze vaccination coverage of children under 1 year old in the State of Roraima, Brazil, between 2013 and 2017, and show health professionals' perception, regarding the barriers that influence the achievement of higher vaccination coverage, 2017. Methods: This was a descriptive study that analyzed vaccination coverage for bacillus Calmette-Guérin (BCG), rotavirus, poliomyelitis, yellow fever, pentavalent, meningococcal C conjugate and 10-valent pneumococcal, based on data from immunization information systems. The barriers identified by the professionals were measured by a questionnaire with Likert scale answer options. ABSTRACT

Objetivo: Desenvolver um instrumento eletrônico capaz de fazer o controle nominal do calendário vacinal da população, alertando tanto o cidadão que sua vacina está para vencer quanto o agente de saúde que atua na área de sua residência, atualizando o modo como é feito o controle atualmente. Método: A criação do software com uma interface minimalista seguiu a metodologia tradicional. Resultado: O instrumento desenvolvido alerta, por e-mail, tanto o cidadão quanto o agente de saúde, mas também emite na tela, ao ser acionado, um aviso sobre a existência de pessoas necessitando ser imunizadas, funcionando, dessa forma, tanto online como offline. Conclusão: Desenvolveu-se um instrumento simples capaz de funcionar até mesmo em localidades desprovidas de internet, que ainda precisa ser implantado em uma unidade de saúde para ter sua eficácia testada.


Objective: To develop an electronic instrument capable of making the nominal control of the population's vaccination calendar, alerting both the citizen that his vaccine is about to expire and the health agent who works in the area of his residence, updating the way the control is currently carried out. Method: The creation of the software with a minimalist interface followed the traditional methodology. Conclusion: A simple instrument has been developed capable of working even in locations without internet, which still needs to be implemented in a health unit to have its effectiveness tested.


Objetivo: Desarrollar un instrumento electrónico capaz de realizar el control nominal del calendario de vacunación de la población, alertando tanto al ciudadano que su vacuna está por expirar como al agente de salud que trabaja en el área de su residencia, actualizando la forma en que se lleva a cabo el control actualmente. Método: La creación del software con una interfaz minimalista siguió la metodología tradicional. Resultado: el instrumento desarrollado alerta tanto al ciudadano como al agente de salud por correo electrónico, pero también emite una advertencia en la pantalla, cuando se activa, sobre la existencia de personas que necesitan inmunizarse, por lo que funcionan tanto en línea como fuera de línea. sin conexión. Conclusión: Se ha desarrollado un instrumento simple capaz de funcionar incluso en lugares sin internet, que aún debe implementarse en una unidad de salud para que se pruebe su efectividad.

ABSTRACT

O estudo objetivou analisar a situação vacinal de crianças matriculadas em Centros Municipais de Educação Infantil da Zona Sul de Natal (RN), para as vacinas pneumocócica 10 valentes e meningocócica C. Trata-se de uma pesquisa epidemiológica e retrospectiva de abordagem quantitativa, realizada a partir de levantamento dos dados de 733 cartões de vacina junto aos 15 Centros Municipais de Educação Infantil na Zona Sul de Natal. Para a vacina meningocócica C, 500 (63,75%) crianças apresentaram esquema vacinal completo, 224 (30,64%) esquema vacinal incompleto, 43 (5,61%) não foram imunizadas e 6 (0,78%) falhas de registro. Em relação à vacina pneumocócica 10 valentes, 470 (60,63%) crianças estão com o esquema vacinal completo, 273 (36,88%) esquema incompleto, 19 (2,49%) não foram vacinadas e 11 (1,42%) falhas de registro. Os achados apontam uma situação vacinal abaixo da meta estabelecida pelo Programa Nacional de Imunização.


The vaccine situation of children enrolled in the Municipal Centers of children education in the south zone of Natal, Brazil, is investigated, with regard to meningococcal c and pneumococcal 10 conjugate vaccines. Current epidemiological and retrospective research was undertaken with 733 vaccine charts from 15 Municipal Centers of Children Education in the south zone of Natal, Brazil. In the case of meningococcal C vaccine, 500 (63.75%) of the children had a complete vaccine chart; 224 (30.64%) presented an incomplete one; 43 (5.61%) had not been immunized and 6 (0.78%) presented blacks in the register. In the case of the pneumococcal 10 vaccine, 470 (60.63%) children had a complete vaccine chart; 273 (36.88%) presented an incomplete one; 19 (2.49%) were not immunized and 11 (1.42%) had gaps in the register. Research revealed a vaccine situação lower than average established by the Brazilian Immunization Program.

ABSTRACT

The objective of this research was to generate the evidence on the private sector's contribution to overall immunization coverage for selected pediatric vaccines in India. Using IMS Health's (now IQVIA) vaccine sales audit data and innovative methodological approach we estimated private-sector vaccine share in the total immunization coverage across selected pediatric vaccines in India. Our estimates suggest that private sector remains an important contributor to immunization services in India not only for Universal Immunization Program vaccines (Bacillus Calmette朑u閞in [19.23%], HiB pentavalent [11.09%], hepatitis B [5.75%], oral poliovirus vaccine [5.48%], Diphtheria-Pertussis-Tetanus [2.66%], and measles [2.17%]) but also for newer vaccines (hepatitis A [4.2%], rotavirus [3.4%], typhoid [3.3%], and pneumococcal conjugate vaccine [2.5%]). As the private sector continues to remain an important access point for immunization services in the country, avenues for potential synergy between public and private sectors should be explored to improve the coverage and quality of immunization services.

ABSTRACT

Abstract This article aims to identify the prevalence and factors associated with influenza vaccination in pregnant women. This is a cross-sectional study conducted in a municipality in the southernmost region of Brazil, which included all women giving birth in 2016. The outcome was having received the vaccine against influenza during pregnancy. Sociodemographic, behavioral and prenatal care characteristics and morbidities were analyzed. The analysis included sample description, the prevalence of vaccination for each independent variable and a multivariate analysis. Two thousand six hundred ninety-four pregnant women were interviewed, of which 53.9% reported having been vaccinated. Factors associated with increased prevalence of vaccination were mother's higher schooling, prenatal care, tetanus vaccination and prenatal care performed in a public service. On the other hand, prenatal care onset after the first quarter reduced the prevalence of vaccination. The results point to the need to reinforce the importance of vaccination against influenza among pregnant women and among health professionals, regardless of the severity of the current epidemiological setting.


Resumo O objetivo deste artigo é identificar a prevalência da imunização contra a gripe em mulheres grávidas e seus fatores associados. Estudo transversal realizado em um município no extremo sul do Brasil, que incluiu todas as mulheres que deram à luz no ano de 2016. O desfecho foi ter recebido a vacina contra a gripe durante a gravidez. Características sociodemográficas, comportamentais, do pré-natal e morbidades foram analisadas como fatores associados à vacinação. A análise constou de descrição da amostra, prevalência da vacinação para cada uma das variáveis independentes e análise multivariada. Foram entrevistadas 2.694 parturientes, das quais 53,9% informaram ter recebido a vacina. Os fatores associados a uma maior prevalência de imunização foram: maior escolaridade materna, realização do pré-natal, ter realizado a vacina antitetânica e fazer o pré-natal em um serviço público. Por outro lado, o início do pré-natal após o primeiro trimestre reduziu a prevalência de imunização. Os resultados apontam para a necessidade de reforçar a importância da imunização contra a Influenza entre mulheres grávidas e entre profissionais da saúde, independentemente da gravidade do atual cenário epidemiológico.

ABSTRACT

Background and Aims:There is a high burden of vaccine-preventable diseases in the children under five years of age, particularly pneumonia diarrhea and which is greatly affected by low immunization coverage despite the existing efforts and policies. This study was carried out in Butaleja district and was aimed at establishing the socio-demographic determinants of vaccine coverage for pneumococcus and rotavirus among under five children (U5C) in the district. Study Design:This was a mixed methods cross-sectional study.Place and Duration of Study:Busolwe Town Council, Butaleja District, Eastern Uganda.Methodology:Structured researcher administered questionnaires were administered to 434 caregivers of U5C in different parts of Butaleja district. In-depth interviews with key informants and focused group discussions with Village Health Teams and community members were conducted. Review of Health Management Information Systems records was done. STATA 15 was used to analyze the data.Results:The study found that there is a declining trend in completion of the doses of Pneumococcal vaccine (PCV) and Rotavirus vaccine. For example, in quarter 1 of 2019, out of the 312 children who started immunization, only 2 completed Rota virus immunization and only 117 completed PCV vaccinations a trend that has been observed since 2016. The factors that showed a significant association with the the fact that they gave their child at least one dose of the vaccinewere having been sensitized on the current immunisation schedule(P-value = <0.001), misunderstanding that vaccine is harmful for child(P-value = 0.007), willingness to take children to vaccination(P-value = <0.001), and social factors such as family (P-value = <0.030). Gender also played a key determinant role where the children’s fathers lacked knowledge on significance of immunization and thus discouraged the mothers from taking the children for immunization. Inadequate funding was also highlighted from the Focus Group Discussions.Conclusion:Vaccine coverage for pneumococcus and rotavirus is still low in Butaleja district mainly due to the attitudes and perceptions of caregivers as well as the knowledge gap. There is need forextensive sensitization of all community members to enable them understand the significance of immunization. It would further be important to increase the funding of the immunization programme to intensify and ensure effectual outreaches as well as the establishment and enforcement of a policy for immunization compliance.

PURPOSE: The vaccination level of rotavirus vaccine not supported by the government is not known. As vaccines not included in the national immunization schedule are not registered in the computerized national immunization registry system, their vaccination rate cannot be calculated according to the same method used in government-supported vaccines. Therefore, this study aimed to measure the status of the vaccination rate of rotavirus not included in the national immunization schedule. METHODS: The target population is the 0-year-old cohort. The survey population was composed of registered children born in 2017 enrolled in the Immunization Registry Information System. The survey was conducted through a computerized telephone survey method. The survey variables were as follows: vaccination order and date, provider, and data source. Factors related to complete vaccination were the child's sex, residence, birth order, and parents' age, educational level, and job status. RESULTS: Children's vaccination rates for the rotavirus vaccine by 2017 were 88.0%, 86.9%, and 96.6% for the first, second, and third doses, respectively. The rate of complete vaccination was 85.6%. The factors related to rotavirus complete vaccination were the child's sex and birth order, area of residence, parents' age and job status, and father's education level. CONCLUSION: In the future, it is necessary to conduct regular investigations on the rate of rotavirus vaccination as a tool for the development of the rotavirus infectious diseases control policy or as an evaluation tool for vaccine programs.

Entre recém-nascidos de risco, a imunização torna-se relevante em virtude do caráter preventivo. A pesquisa objetivou analisar a situação vacinal dos recém-nascidos de risco de Cuiabá-MT, bem como a satisfação e dificuldades das mães em relação à imunização. Estudo descritivo, quantitativo, com 113 recém-nascidos de risco, selecionados pela Declaração de Nascido Vivo: 25,7% apresentaram atraso vacinal, sendo a falta de vacinas na Unidade de Saúde (50,0%) o motivo mais relatado. Das crianças em atraso, 65,5% tinham mais de uma vacina atrasada, 100% apresentavam peso abaixo do esperado, 75,9% eram acompanhados em seu crescimento e desenvolvimento pela Unidade de Saúde, 69,0% apresentaram episódio de doença nos seis primeiros meses de vida, 41,4% das mães eram menores de 18 anos. As mães relataram várias dificuldades quanto ao serviço de imunização. Destaca-se a importância de estratégias que visem melhorar a qualidade no atendimento, estrutura dos serviços de saúde, acolhimento e preparo dos profissionais.


Among newborns at risk, immunization becomes relevant due to its preventive characteristic. The research aimed to analyze the vaccination status of newborns at risk of Cuiabá-MT, as well as satisfaction and difficulties of mothers regarding immunization. This is descriptive study, quantitative, with 113 newborns at risk, selected by the Statement of Live Birth: 25.7% had delayed immunization, being the lack of vaccine at the Health Unit (50.0%) the reason most often reported. Of the children with delayed immunization, 65.5% had more than one vaccine late, 100% had weight less than expected, 75.9% were accompanied in their growth and development by the Health Unit, 69.0% had an episode of illness in the six first months of life, 41.4% of mothers were under 18 years. The mothers reported more difficulties to the immunization service. We emphasize the importance of strategies to improve the quality of care, structure of health services, reception and professional preparation.


Entre los recién nacidos en situación de riesgo, la inmunización se vuelve relevante debido a su característica preventiva. Esta investigación tuvo como objetivo analizar el estado de vacunación de los recién nacidos en riesgo de Cuiabá-MT, así como la satisfacción y las dificultades de las madres con respecto a la inmunización. Este estudio descriptivo, cuantitativo, con 113 recién nacidos en riesgo, seleccionados por la Declaración de Nacimiento: 25,7% había retrasado la vacunación, siendo la falta de vacuna en la Unidad de Salud (50,0%), la razón más frecuentemente reportado. De los niños con retraso en la vacunación, 65,5% tenían más de una vacuna retrasada; 100% tenían un peso inferior a lo esperado; 75,9% fueron acompañados en su crecimiento y desarrollo en la Unidad de Salud; 69,0% tuvo un episodio de la enfermedad en los seis primeros meses de vida; y 41,4% de las madres eran menores de 18 años. Las madres reportaron varias dificultades relativas al servicio de vacunación. ABSTRACT

As infecções respiratórias constituem um conjunto de doenças comumente relacionadas aos idosos, considerando que o vírus da influenza é um dos principais agentes etiológicos. A vacinação desses indivíduos é considerada pela Organização Mundial de Saúde como a mais efetiva estratégia para a redução da morbimortalidade pela doença. O Brasil tem buscado nesses últimos anos, como meta vacinar 80% da população alvo. Este estudo objetivou relacionar a cobertura vacinal da influenza e a taxa de mortalidade por doenças respiratórias em idosos. Trata-se de um estudo transversal com análise secundária de dados. Os dados sobre cobertura vacinal no ano de 2010 em idosos dos 496 municípios gaúchos foram obtidos na página do Programa Nacional de Imunização e de mortalidade no Sistema Informação de Mortalidade. Os resultados evidenciaram que 49% municípios gaúchos atingiram a meta de 80% de idosos vacinados, nos municípios com cobertura vacinal abaixo da meta o número de mortes foi de 5,2 por 1.000 idosos, média essa significativamente maior que nos municípios com cobertura igual ou maior que 80%. Concluímos que a meta proposta pelo Ministério da Saúde Brasileiro de vacinar 80% ou mais de idosos é eficiente para a diminuição da mortalidade por doenças respiratórias.


Respiratory infections are a group of diseases commonly related to the elderly, since the influenza virus is one of the main etiological agents. Vaccination of these individuals is considered by the World Health Organization to be the most effective strategy to reduce morbidity and mortality from the disease. Brazil has sought in recent years to vaccinate 80% of the target population. This study sought to relate the vaccination coverage for influenza and the mortality rate from respiratory diseases in the elderly. This was a cross-sectional study with secondary data analysis. Data on vaccination coverage in 2010 of the 496 municipalities of the elderly in Rio Grande do Sul were obtained from the website of the National Immunization Program and mortality in the Mortality Information System. The results showed that 49% of municipalities reached the target of 80% of seniors vaccinated. In municipalities with below target vaccination coverage, the number of deaths was 5.2 per 1,000 elderly. This average is significantly higher than in municipalities with coverage equal to or above 80%. The conclusion is that the target proposed by the Brazilian Ministry of Health to vaccinate 80% or more of the elderly is effective in reducing mortality from respiratory diseases.
